VANFLYTA4U Patient Support
VANFLYTA4U provides support and information to help your patients access VANFLYTA, which is available through Biologics and Onco360.
Biologics by McKesson and Onco360 Oncology Pharmacy are both included in the Limited Specialty Pharmacy Network. Please fax the Patient Enrollment Form and your patient's prescription directly to Biologics at 1-800-823-4506 or to Onco360 Oncology Pharmacy at 1-877-662-6355.
Upon receiving the form, the specialty pharmacy will be able to:
- Conduct a benefits investigation
- Help complete the prior authorization
- Assist with enrollment into the VANFLYTA Copay Program for eligible commercially insured patients
- Refer uninsured patients who may be eligible for the VANFLYTA Patient Assistance Program to VANFLYTA4U
- Provide you with a summary of benefits when requested
